Abstract

Even with completely normal looking articular cartilage, patients may experience intractable anterior knee pain. Most of these patients get better with modification of activity, core stability training, and non-operative measures managed by a non-operative specialist. One must be alert to signs of catastrophizing and mental illness. In more extreme cases, when objective cartilage softening is demonstrated, an offloading surgical procedure may be appropriate and can be very effective when all other options have been exhausted.
