Perforated femtosecond laser-created flap

Abstract

The incidence of flap-related complications with a mechanical keratome is about 5% and includes flap tears, buttonholes, free flaps, and decentered flaps [1]. The creation of a buttonhole does not specifically occur with the femtosecond laser; instead the laser can create a unique type of flap complication: vertical gas breakthrough. With early recognition, patients can do well despite this complication. However, if this complication is not recognized and the flap is lifted, the femtosecond flap vertical gas breakthrough may be converted into a buttonhole.
