Abstract

A major redesign of health care and education is underway that promotes mutual learning to import the transformative redesign of the scope and process of care, the workforce and institutions that improves population health, engages people and communities, and improves quality while reducing the per capita cost of care. The National Center for Interprofessional Practice and Education was funded via a national competition by Health Resources and Services Administration and private funders to gather data that will produce information and evidence regarding team composition, training, and function in achieving outcomes and their environmental success factors. The center will inform, connect, engage, and advance the various stakeholders who are able to make the transformative redesign with sustainable change in an environment of continuous improvement.
