Abstract

Total pancreatectomy with islet autotransplantation has been increasingly used over the past decade to treat children with refractory pain and lifestyle limitations from chronic and recurrent acute pancreatitis. Children undergoing the procedure require multidisciplinary support in the postoperative outpatient setting in order to achieve optimal outcomes. This includes the management of new (sometimes temporary) diabetes mellitus, postoperative pain and weaning off of opioids, exocrine pancreatic insufficiency, and optimizing nutritional status. In this chapter, the authors present practical considerations for the postoperative care of pediatric TPIAT patients and review the current literature surrounding short- and longer-term outcomes specific to this patient population.
